TERMS & CONDITIONS FOR CONSUMERS
FIREARMS FAX Reimbursement Guarantee claims (each a "Claim") may be submitted per,
and are subject to, the following Terms and Conditions.
FIREARMS FAX agrees to pay to the holder of a FIREARMS FAX Gun History Report
(the "Report") making a Claim (the "Claimant") the Claimant's purchase price of the gun to
which the Report relates (the "Gun") to a maximum of Five Hundred Dollars ($500.00) if
the FIREARMS FAX Gun Report states that the Gun was not stolen.
The following additional terms and conditions apply to the Reimbursement Guarantee:
1. The Claimant must have registered for the Reimbursement Guarantee within thirty (30)
days of purchasing the Gun. Registration may be madeonline at www.FIREARMSFAX.com.
Registrations can also be made by mail postmarked within (30) days from the date of
purchase by sending name, address, telephone number, email address, make, model,
caliber/gauge, the Serial Number of the guaranteed Gun and a copy of the original receipt
(with the NIC approval number) from an FFL Dealer to:

2. The Claimant must own the Gun at the time the Claim is made and must provide proof of
ownership of the Gun in the form of dated sales receipt with the purchasing store’s name
and address. Handwritten receipts will not be accepted.
3. The Claimant must complete and submit to FIREARMS FAX an executed and notarized
Affidavit Claim Form and provide a complete copy of the FIREARMS FAX Report for the
gun in question. The Report must have been run prior to the date the Gun was purchased
by the Claimant.
4. The Claimant must submit a Police Report from the jurisdiction where the gun was stolen
indicating the serial number, make, model, caliber/gauge, address and date where it was
stolen. This applies only to the fifty contiguous states, territories not included. This report
must be dated prior to the purchase of the firearm.
5. If, prior to purchasing the Gun, the Claimant knew, or should have known, that the Gun
was stolen, FIREARMS FAX reserves the right to reject the Claim.
6. FIREARMS FAX will pay the purchase price up to a maximum of Five Hundred Dollars
($500.00).
7. Claimant’s reimbursement will be offset by other coverages available to claimant,
including any insurance or coverages available through the purchasing gun shop.
8. FIREARMS FAX will accept only one Claim per Gun IdentificationNumber under the
Reimbursement Guarantee, or any other FIREARMS FAX certification or guarantee
program.
9. Each household may submit only one Claim per calendar year.
10. The Reimbursement Guarantee will expire one (1) year after the date of the first Report
run by or for the Claimant.
11. The Reimbursement Guarantee is valid solely for the Claimant, and may not be
transferred or assigned to any third party. If a third party desires coverage under the terms
of the Reimbursement Guarantee, he/she/it must register the firearm for the
Reimbursement Guarantee.
12. Employees of FIREARMS FAX and its affiliated companies, and members of their
families, are not eligible.
13. FIREARMS FAX may, at any time and without prior notice, change the terms of the
Reimbursement Guarantee, or discontinue the Reimbursement Guarantee.
14. The Reimbursement Guarantee will be governed by the laws of, and Claimant consents
to the exclusive jurisdiction of the courts of, the Commonwealth of Pennsylvania, United
States of America.
